What is a Penile Prosthesis?
A penile prosthesis (also called a penile implant) is a surgically implanted device placed inside the penis to allow men with ED to achieve an erection on demand. It is entirely concealed within the body and undetectable when not in use. It does not affect urination, orgasm, or ejaculation.
Types of Penile Prosthesis
Inflatable Penile Prosthesis (IPP) — 3-piece
The gold standard. Two inflatable cylinders are placed inside the penis, a fluid reservoir behind the abdominal wall, and a small pump in the scrotum. Squeezing the pump transfers fluid into the cylinders to create a natural-looking, rigid erection. Releasing a valve deflates the device when not needed.
The 3-piece inflatable prosthesis produces the most natural appearance and the firmest erection. It is Dr. Kalpesh Kapadia’s preferred implant for most patients.
2-piece Inflatable Prosthesis
Similar to the 3-piece but the reservoir is integrated into the pump. Slightly simpler but produces less rigidity and flaccidity compared to the 3-piece device. Suitable for selected patients.
Semi-Rigid (Malleable) Prosthesis
Two flexible rods are inserted into the penis. The penis is always semi-rigid and can be bent into position for intercourse or bent downward for concealment. Simpler surgery, lower cost, but less natural appearance.
Who is a Candidate?
A penile prosthesis is recommended when:
- Oral medications (Viagra, Cialis) have failed or are medically unsuitable
- Intractable ED is caused by diabetes, cardiovascular disease, or previous pelvic surgery
- ED is severe and persistent, significantly affecting quality of life
- The patient has Peyronie’s disease with associated ED

The Surgical Procedure
The surgery is performed under spinal or general anaesthesia and takes approximately 45–90 minutes. A small incision is made at the base of the penis or in the lower abdomen. The cylinders, pump, and reservoir are placed entirely within the body.
Most patients stay in hospital for 1–2 days. The device is activated for the first time 4–6 weeks after surgery, once healing is complete. Normal sexual activity can typically resume at 6–8 weeks.

Will anyone be able to tell I have an implant?
No. When deflated, the 3-piece inflatable prosthesis is completely undetectable. There is no visible external component.
Does it affect sensation or orgasm?
A penile prosthesis does not alter sensation or the ability to orgasm and ejaculate. It only provides the mechanical rigidity needed for intercourse.
What are the risks?
